Immunization Education Session

Parents seeking non-medical exemptions for their school-age child(ren) (ages 4-17) must attend an online information session first before completing the Statement of Conscience or Religious Belief form. The information provided is to assist parents to make an informed decision. You need one Statement of Conscience or Religious form for each child but only one education certificate for your family. The Immunization Information Sessions are free. 

We do not provide specific medical advice.  All sessions will take approximately an hour.

Registration:

• Registration is required.
• Registration is on a first come basis. 
• To register online you must have an e-mail address and be the parent attending the session.

Parents/legal guardians arriving 10 minutes late or more to the education session will not receive a Certificate of Attendance.
